That's easy - OOPS! Mistakes are bound to happen, but we have plenty of safeguards to try to prevent that. Even so, some have been executed. Oh well, many more have been released who were guilty, but because the officers didn't read some text to them, they walked, even in murder cases. So, those balance it out.

We are all slogging toward the box, or the urn, or whatever recepticle your remains may be located, so it's really just about taking some of your time away, isn't it.
