Some of them don't. The ones I looked at recently had cup holders where you could put an ashtray (form fitted to go there) in one of them and you had to pay for the ashtray if you wanted one. I actually asked because I noticed there wasn't one and I thought it was unusual. I only looked at small cars though and most were imports other than Saturn so maybe they are still common in American cars?
