I think you should look at Microsoft´s HTML Help. I am using it and I think it does all you need.

1. I write separate HTML pages (documents) for each program. I include as many BMPs as I want, and I use style sheets to keep all pages similar.

2. I print user documentation through a master document, which links (includes) all the separate pages.

3. I use the same HTML pages with the HTML Help program in order to provide on-line help. (This needed some --minimal-- coding in my base FORM class.)
