Add the class to the VFP Toolbox - Tools/Toolbox

Then when you need a form based on this class, Rt click on the class in the toolbox and select "Create Form."

Or

Open it in your class browser, select the form class, and drag the icon (top right of class browser) and drop it on your VFP desktop. If it asks for a table just click cancel and you will have the form.
