The report has page header and page footer, but there is also a separate report header and report footer, that only appear once for the entire report. You have to activate it under "Report | Title/Summary".

In some cases it also helps to use a "Group Footer" that spans the entire report. In other word, put a group expression like ".T." that doesn't change throughout the report.
