Use the IShellLink interface to create links on the desktop. There are several examples on it in the MSDN. George Tasker also has a FAQ on how to use it from VFP (you can find it in the FAQ section of the VFP forum here on UT).

I don't know about the Start menu. Probably you have to create a .lnk file in a special directory. I believe George Tasker has something about it in his FAQ.
