The insurance company did. But in the end the collective did. Everyone pays (or at least is required to pay) for the collective.

Up here there are different levels of coverage, and there are different rules per insurance company, so it is not totally fixed for you, but at least there is a firm basis for everyone that covers all life threatening treatments. If you want something better, you always can choose a private clinic.
