Kevin,

click on your setup project to select it. Then in the top of the solution explorer there is a button for "File System Editor". click that button and you will get the file system editor. In the left pane, click on "Users Desktop" and on the right side right click and choose Create New Shortcut. It will give you the opportunity to navigate to the primary output of the build. After you create this if it is selected and you go to the property window, the target should say "Primary Output". After building and installing, the shortcut will be created. I do this with all my projects and you can set the icon that will display for the shortcut.

For the setup project there is also a button for the user interface editor. This is where you choose what the user can customize during the install process.
