Another way to look at it is that they wrote the bill and lowballed the number of enrollees in order to claim a less dramatic basis so the program wouldn't seem nearly as costly as reality dictated. Why would you do that to allow a program to run for only 7 months? Why would you create an example that opponents will use to try to prove that socialized schemes don't work? Occam's Razor says that this was a dumb screwup not a nefarious scheme.
Absolutely not! The estate should be billed for services rendered. So you're advocating a system where policing services should be covered by taxes unless you need/cause additional services in which case you should have to pay. Why can't that be applied to healthcare too?
"... They ne'er cared for us
yet: suffer us to famish, and their store-houses
crammed with grain; make edicts for usury, to
support usurers; repeal daily any wholesome act
established against the rich, and provide more
piercing statutes daily, to chain up and restrain
the poor. If the wars eat us not up, they will; and
there's all the love they bear us."
