As I said, of course I feel very sorry for anybody who thinks they're going under for a simple procedure and wakes to find their life changed forever or (worse) doesn't wake. In this case I see that there are 3 investigations underway so we need only wait to find out whether there is blame or improvement to be sought and if so, where.
