Yes, if you are under 65, you pay monthly and over and above that you may hire a doctor and have a job done at your own expense. I know a fellow who wanted a different kind of shoulder operation than his doctor recommended. He didn't know that doctors here are better trained than doctors in the USA so he went there and had it done.
