You can pay if you want - there are resellers. It's a cultural thing; people appreciate it more if they pay for it, so there's a certain kind of artists who repackage and sell stuff that is otherwise free. The usual path is to download an .iso image of a distribution, fry a dvd from it, boot from that dvd and then follow the instructions. Nowadays you don't need to know much; some four years ago you'd be prompted with things like setting partitions and assigning them to mount points in the filesystem. The system updates also don't mean just system, it covers all the apps - amazingly, the installation is standardized and the installer is pretty much a part of the OS (at least from user's POV), so only packages are distributed (there's no setup.exe with its runtimes etc). Getting all your apps updated is pretty much two clicks and one password entry.
If you pay, you get a book, a bootable/installable disk etc. If you don't, you got the same stuff online.
