- Make sure your video drivers are up-to-date and exactly correct for your hardware
- If applicable, try disabling Desktop Composition for that app (IIRC there was a recent thread about this)
- If neither of those work, try with SP2 and the latest hotfixes installed (i.e. 7423). I had a client with a Vista machine, running an SP1 app where form titlebars and the controlbox/min/max/close buttons weren't always appearing. It was fixed by going to SP2/7423.
