While the "ask the user" approach (which includes "oh, we'll just make it an option") is absolutely necessary, it's not sufficient. A good UI designer knows how to design a UI better than a user, just as an architect better knows how to design a house than its occupant.

We must take the users' wishes into account, but this ain't a democracy. :)
