I have a Compuserve account and I could mail you the files you need, but I went down that path last year and it was not too pretty. Codebook 3.0 was published in 1995 and there were some serious problems. The errata files help, but there are still problems. After struggling for a while, I discovered the Mere Mortals framework which is based on the original codebook. They have improved the framework dramatically over the last 3 years and eased the learning curve.

My suggestion would be to look on the UT files section for Mere Mortals or visit www.oakleafsd.com for more info.
