The issue is with wearing an actual flag, not the design of a flag. You cannot wear a U.S. flag or treat it in any way that may be disrepectful. Back in the 60s (gee I'm getting old :o) I had a pair of pants that had the flag design on them and all of my relatives freaked out when I wore them to a family get-together. Of course, they would have freaked out had I worn anything other than what they considered appropriate dress :o) My mom liked to stir up trouble and she put me up to it. :o)
