I haven't had one, but I've had a fair amount of dental work. I suspect it's the general issues around dental work. Having anesthesia, the sound of the drill (which for me is the biggest issue-I try to remember to bring an iPod when I'm going to have drilling), keeping your mouth open for a long time (I find I'm sore the next day from that), and so forth.
