You are not going to like my response :( While I agree with one response that any changes/improvements should have been billed separately (and you can still do that), I disagree on whether or not he received what he paid for. If one of the key requirements (spelled out in the contract) does not function correctly or reliably, then he did not get what was in the contract and deserves a refund for the product (400.00 unless you can get it working reliably or prove that the cause of the malfunction is entirely outside of your app) regardless of the number of hours you put into it. Sorry. A lot depends on the wording of your contract though.
