Specifically, it's probably an error _in_ the form. Not in the menu. But you probably realize this right? Anyway, I'm no registry expert, but I have hard time believing it's due to the registry. But, to diagnose that you could have the user try this on another computer. W/O more info it's pretty hard to tell.
