Aren't exe COM servers usually registered by running "myExe.exe /regserver" instead of through regsvr32?

If you want to add it to the open with list, you could use the registry key indicated in http://support.microsoft.com/kb/258860 and try adding a new entry. I haven't tried it myself, so I don't know if it will work.
