If this applies to all icons and all folders, I'd suspect that my display driver was corrupted. You could try reinstalling the driver from Control Panel/Display/Settings/Display Type.

I'm not sure that this will help, but try it anyway. Install TweakUI (one of the PowerToys, available for download from Microsoft) and run it (it will create a Control Panel applet.) Select the Repair tab, and hit the Rebuild icons button, which will fix up registry entries related to icon images.

The help for the Repair/Rebuild Icons reads as follows:

The "Rebuild Icons" button rebuilds all Explorer icons and removes unused Explorer icons from memory.

Use this button when you find that Explorer is displaying the wrong icon for a program or shortcut.
