Perhaps you might consider another approach. If your objective is to create a formatted image for faxing, then you would be better off printing to a "fax printer". This is the same process as you would use if you wanted to send a fax of a Word Document. All fax programs I've used install a printer driver that you can print to. Some will allow you to send the resultant image to a file instead of the fax server.
