(1) My hardware configuration has not changed in many months -- probably more than a year.

(2) It happens on many forms. And not necessarily the first time I work on it; I can be modifying and testing it, opening and saving it repeatedly, and it will suddenly fail.

I have seen no pattern whatsoever in the problems.
