I've seen this happen - but rarely and have never discovered a repeatable scenario. But it is very rare and I've always found that closing all the windows or rebuilding or closing and re-opening the solution brings things back to normal. I assume that the IDE somehow loses the track to the source location.

Are you trying to navigate to a definition where the source resides in the same project, another project in the solution or a referenced DLL ?
