If you're using SQL Server 2005, then I suggest to remove all the subqueries and use one OUTER APPLY to get all extra e-mails.
For now, get rid of all these subselects and see, if the performance will be better.
Post sub-selects separately, I'll tell you how to change your query.
If it's not broken, fix it until it is.
