You've hit the nail on the head.
IMHO: In the end, healthcare administration is about rationing. It's also about maximizing arrival of dollars at the coal face. It's also about motivating and rewarding those in what used to be called "vocations", because you will find that nobody can be a better guardian of precious funds, even allowing for the inevitable rogues in their ranks.
If you start and end with that, everything else becomes clearer. IMHO.
