I think websites put up forms in that state and then have code to suck out the data, essentially leaving the blank form.

I requested to know if they sent us the flattended or fillable format. Or, if they simply have a PDF page with no form fields, but just a simple layout where the user enters the data, there I'm afraid there isn't much we could do about it.
