I surely was surprized by the:
In 61% of these cases (4,941 patients), the intentional overdose was given without the patient’s consent.(14)

But I guess a situation like me and my father would be counted as "without the patient’s consent",
as I, not my father will decide. Simple set theory with perhaps partially loaded formulation.
With my father it is more than DNR - we watched my mother die of cancer and afterwards
talked long about such possibilities. Chances are that there will be other reasons,
but a dose of morphine was certainly talked about.

For me fulfilling his personal wishes is tantamount. If he would ask me to provide the means,
I would talk with him and after a small waiting/cooling period would provide him with things I can
legally get him - and there are quite a few things on the market and in prescription drugs -
while assisting during the deed might get me in trouble, providing stuff will not.
And I do believe for such situations laws like in switzerland or netherland
are better, as he might fumble it.

But to get back to the issue:
Did you look deeper ? Were those cases purely on doctor decision ?
