Maybe the icon cache got corrupted or the original desktop shortcut wasn't pointing to the actual exe but to a shortcut on the start menu? Or it was stored in a user's profile and not set for all users? Deleting the icon cache so it gets rebuilt usually works:
C:\Users\User\AppData\Local\IconCache.db
