I reindexed a combined index and this has resolved the issue. This is based on my testing. The command that was having a problem on that index was taking 34 seconds. Now, it takes 0.000. However, there about 530 SQL commands to obtain that page. So, it is normal this type of transaction takes between 2 to 4 seconds.
