One of those was about asking the user if they want to do something. "Are you sure you want to delete this record?" and then have yes/no buttons. This kind of thing doesn't exist in real life. We get so many of these dialogs and people simply don't read them. They just hit the button, then go, "Oh Sh**!" His point is the question is not needed. Provide a way to undo something. (Example: How GMail handles deleted mail)

The one that annoys me most is the one that asks "Are you sure that you want to leave?"
