I had a similar problem in 3.0 and from what I remember, record #4 is the data environment. You may have to resort to hacking the SCX, but I wouldn't try that until I made a backup copy (no matter how messed up a file is, it can always be messed up worse).

It sounds like the problem isn't the form, but the table it's trying to open in the data environment. Try browsing the SCX file to ID the table and see if you can open that table(s) from the command window.
