As others have said, don't store the file in SQL Server. AFAIK, Flash is not supported on iPad.

As far as "saving" I would prefer (as I mentioned to Michel) to convert the phone as well as the entire form (on Submit) to a PDF. Then "do something" with the PDF; maybe save it to SQL or maybe just store it on the drive.
