1) Yes, storing images this way is the right way to go
2) They should be able to mail it to themselves. For apps, you'd need different versions, one for each phone. Go the easy route and just have them upload the pic.

FYI, in some states, it may be illegal for the employer to post the employee's picture, even on an internal intranet site. I know when I worked at a Fortune 100, they couldn't post our pic without permission because of this.
