For me, that happens when:

1. I log on to UT
2. After navigating away, I go back to UT and log on a second time

It seems in that case the user name is getting pulled from the cookie and is populating that field. That didn't happen in the past. Yes, that populated user name is upper case but that doesn't seem to be a problem.

The whole thing seems to be a side effect of the UT nearly unique characteristic of not being able to log off. The logon stays alive until the browser is closed.
