Oral Roberts, in the early 80s. I remember because it was on the radio in the morning as I was going to my dentist in San Franscisco. I was ranting about it pretty seriously as I got in the chair. (I was much less shy about telling people what I think in those days <s>) As he was preparing the needle for the novacaine and while I was about halfway into a tank of nitrous he told me he was a graduate of Oral Roberts University, Of course I thought he was kidding. He wasn't.

In fairness, he was a good dentist, but I never was able to go back to having a medical procedure of any kind done by somebody who thought the earth was 6000 years old.
