Ok, several people here have posted examples of using
MAPI to send email.

But it appears to me that MAPI is not actually doing any
sending at all - rather it manipulates the inbox of Windows
messaging.

Is there any way to force MAPI to 'go online now and send'
a message?

Along the same lines 'go online and get messages now' and
then be able to see just the new unread messages?
