I don't know if I'd understand the exact problem without looking at the code itself, but my first impression is either the Init method returning False (and aborting the object creation), or else something's killing the form prematurely (Unload, Destroy, Release).

I'd step through the Init once if I were you. Hopefully the problem lies there. :-)
