REPORT FORM makes use of the current Windows printer driver. Several things about it might cause problems like that:

1. If the driver is not exactly correct for the actual printer hardware
2. Maybe the driver is old and buggy, there might be a newer one available for download
3. Driver misconfiguration e.g. configuring the driver for 2MB printer RAM when it only actually has 1MB
