The forms I have been using allow for only ONE selection of a series of choices then on that selection do an action.

Is there a similar form that would allow me to make multiple selections and accumulate the properties of each of those selections to do an action.

For example I want to have the operator be able to create a list in which he selects what information is contained on the list
names, addresses, telephone numbers, email addresses, selected from a list of 10 other bits of data he can include or exclude

What I am looking for is a sample code that will allow me to build on.
