I use InstallMate which provides a very cheap and usable platform for building installs.
What I like about it is that it has a builder Wizard that's powerful and lets you do most things interactively, plus an easy project format that allows you to customize if you need to. Everything is customizable, it's easy to hook up and fire custom actions, and the UI of the installer is easily customizable.
It also builds very lean installs and doesn't use the MSI format internally so it has no requirements for Windows Installer runtimes which makes for very small deployment footprint - basically the size of your compressed files plus about 500k.

And no I get no kickback from them - just one of those companies that does one thing and does it really well. And their support is awesome as well - the developer is very responsive to questions, comments and suggestions.
