FWIW, when I went to my primary for something the other day, they handed me a page showing what meds they had on record for me to correct. My eye doctor reviewed the list with me when I was in the other week, though didn't have access to my primary's list. (I confuse everyone by seeing doctors associated with two different hospital systems. I know the one my primary is in does share my records among their docs.)
