Never ran into this - usually I get questionaires which I cannot edit in reader (my version of Acrobat is somewhat obsolete...) and I type my info in in Foxit.
Perhaps a special PDF form (dunno how well the old .fdf is still supported) - ask for usual way of work ;-)
