One can live a long live with it. But you need to be stern.
Note that sugar is not meant as the food industry is using it. It basically means carbohydrate - wheat flour is part of it.
Friend of mine gots good results with high basal metabolism. He started a career as a runner. From what your pictures showing he started with more mass and havy smoke. (he skipped smoking too) He is now high muscle / fat ratio. That helps a lot.
He put a fortune in measurement. Doing just the right dose of insulin.
And he was upset as he figured out that he have to skip sausage - because of the "sugar" in it ... but he is stubborn. Likes to see his kids growing up.
