After I installed Adobe Reader, I was able to add signature from the saved image to the document, but I still was not able to fill in the form.

In order for you to be able to fill in the data in a PDF in Adobe Reader, it has to be set up as a fill-in PDF with form fields already in it. If you want to save the PDF with the data fill-in, the PDF needs to have extended reader features enabled.
