>>So, the people in the loop are doing a lot of unnecessary work and they and the consumers are enduring stress that adds no value.
If the MD could have seen my plan, he could have ordered one that was there, or if there were none, told me that I'd have to pay for it.
I could have decided that in his office.

That's one of the promises of Electronic Health Records- that coverage can be flagged at prescription time, for your advantage. Plus elimination of this sort of inefficiency is an important expected contributor to the reduced ACA cost Obama talks about. From memory, close to half of the projected $ benefit comes from EHR. So it's great that your doc has an electronic system and next time you go see him/her, maybe it'll work as it ought.
"... They ne'er cared for us
yet: suffer us to famish, and their store-houses
crammed with grain; make edicts for usury, to
support usurers; repeal daily any wholesome act
established against the rich, and provide more
piercing statutes daily, to chain up and restrain
the poor. If the wars eat us not up, they will; and
there's all the love they bear us.
"
-- Shakespeare: Coriolanus, Act 1, scene 1
