My point is that what the MD deems to be "necessary" is more a function of what he can collect for it than its efficacy.

Of course it is. There is no limit to what these voodoo practitioners will try to persuade people that they "need."
I find a parallel with the Catholic church during the middle ages, where the clergy was able to mask its voodoo as dogma for centuries.. until someone woke up and said.. "this is nonsense!"
Each day I say a special prayer for all those Catholics spending eternity in hell for eating one lousy hot dog on Friday before Vatican II said it was OK.
Anyone who does not go overboard- deserves to.
