Because of the impossibility of how. Whoever proposes single payer solution is immediately accused of socialism, in a true communist manner (not a paradox at all - that's called "labeling" and pretty much puts the labeled party into defensive/defenseless position while not requiring any proof from the attacker's side; various Party purges employed the technique copiously).

So everybody knows this won't work as is, and that even oCare isn't the solution - instead "we will provide care for everyone" it's "everybody must pay insurance", fulfilling what wasn't promised but what was achievable. Now the health industry is more powerful than the political system, and on the side of the latter there's no firm will to change anything much.

For every "what? - this!" there's presently a dozen "can't!"s and "won't work!"s, because of all the entrenched and powerful interests.

Offer a parallel payor in the shape of a state insurance ("c'mon, you can't be afraid of that kind of competition, everybody knows it costs more when done by the government, you know in advance it will fail, so give it a sporting chance" may work as a pitch)?

Rebuild the system from scratch?
Actually, scratch that. Replace the industry with a system, that would be it. Keep the industry as competition to it.
