Very sadly, I once saw something very similar to that, and in this country (a fairly well-known organization). It's like taking the concept of "building to a spec" to an insane level. Fortunately, I wasn't around long enough to pull all my hair out - I left before the insanity hit.

I realized there is heightened sensitivity to the following word, but when I hear situations like the one Nick described, I think of the line from the movie "Tropic Thunder", that goes, "Never go full retard". I use it to describe adults who should know better, but wind up outsmarting themselves by implementing a totally brain-dead, idiotic solution. The one here sounds like that.
