The full title of the book is "Being Mortal: Medicine and What Matters in the End" written by Atul Gawande (surgeon at Brigham and Women's Hospital in Boston). IMHO, excellent book as far as the topic and the writing.
Another book by Atul Gawande I would recommend (but more for those who are interested in medicine in general) is "Complications: A Surgeon's Notes on an Imperfect Science"
